A one-page printable covenant card. The learner's name, today's date, and a signature line at the bottom. Above the signature, five short blocks. One for each Core Value. Each block carries the Core Value shape, the Core Value statement, and three lines for the learner to write a specific commitment in his own hand.
The card is designed to be printed, filled in by hand, photographed, and uploaded in the next step. If the learner prefers to type, the same content is provided on the second page of the PDF as a fillable template.
